Responsibilities:
– Provide care, support, and advice for clients at home
– Ensure the care process is followed
– Case management and conducting needs assessments
– Create, adjust, and evaluate care documentation
– Conduct counseling sessions
– Accurate recording of service data
– Collaborate with relatives, doctors, hospitals, nursing homes, and other specialist agencies
Qualifications:
– Completed training as a certified nursing specialist (DN I/DN II/HF/FH or equivalent)
– High social, methodological, and professional competence
– Very good German skills in spoken and written form
– Up-to-date IT skills (MS Office, Perigon)
– Driver’s license category B is an advantage
– Independent and responsible way of working
